An employee was vacuuming up media from a vertical tube dryer while standing inside a plastic collection container while a co-worker was outside of the container with a shovel trying to loosen up the media still inside the dryer. The media inside the dryer became dislodged and entered the collection container, engulfing the employee up to the bottom of his hips. The employee suffered first and second degree burns to both of his legs, just below the hips.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
