Final narrative
An employee was pulling on the fifth wheel release of a tractor trailer when he slipped and fell backward. He landed on the parking lot pavement and suffered a fractured left hip.
An employee was outside assisting a driver in backing a trailer up in a parking lot when their right ring and middle fingertips became caught between the trailer and the wall of the store, resulting in amputations.
An employee was in the trailer inspection lane at a facility to have his trailer inspected for suitability to load. There was a small amount of debris inside the trailer. He climbed into the trailer, collected the debris, and put it at the rear of the trailer for disposal. He then climbed down from the trailer and stepped onto the ground. The pavement was wet and sloped toward a drain, causing him to lose his balance and fall to the ground. He attempted to catch himself with his left hand and arm, and his left wrist was fractured as a result of the fall. He was hospitalized and had surgery.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
