On December 15, 2023, the injured employee was working from a ladder. A co-worker climbed the backside of the ladder and the ladder failed. The injured employee fell approximately 8 feet to the ground and sustained multiple fractures.

An employee was installing grout using a shovel when the precast hollow-core floor gave way, causing them to fall approximately 8-10 feet. The employee sustained a back fracture.

An employee was assisting with assembly of a two-level scaffold. The scaffold fell, causing the employee to fall 8 feet from the second level to the concrete surface below. The employee sustained fractures to the face, jaw, and both wrists.

An employee was repairing an egg collector when the platform collapsed and he fell 10 feet to the concrete floor. The employee sustained a fractured left leg.

An employee was descending a ladder when it collapsed, causing him to fall approximately 8 feet. The employee was hospitalized with a compound fracture to his left leg and a left wrist fracture.

An employee was preparing a body when they slipped on some water and fell. The employee sustained a broken hip and leg.

An employee was descending stone stairs from a stage inside a church. They bent down to pick up a vase, missed the last step, and fell to the ground. The employee was hospitalized with fractured ribs.

An employee was operating a hydraulic lift that is used to lift caskets. The lift fell on him and caught the employee's arm between the frame and an expanding component. The employee sustained a laceration to his head and a puncture wound to his right arm resulting in a fracture.

An employee was on a ladder removing popcorn ceiling material that was 10 feet high when he fell off the ladder to the carpeted floor. He sustained a fracture to his left humerus requiring surgery and hospitalization.

An employee was walking down the stairs with a laptop when they missed a step and sustained a right leg injury that required hospitalization.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

Two employees were working to change a tire on a mobile slasher saw. The slasher weighs around 2,000 pounds and is towed. Employee 2 was lifting the saw while the injured employee was placing a block of wood under the frame for support. The slasher then lowered onto the block and the injured employee's left thumb was crushed between the slasher frame and the wood block, leading to an amputation at the first knuckle.

An employee was using a metal shear when it amputated the tip of his right index finger.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was standing on a step stool, removing the nuts and bolts from the frame of a solar panel that was being replaced. The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.
