An employee was cutting open 50 lbs. bags of solids and emptying them into a screener with the use of a lift table and vacuum lift. The employee was hospitalized that evening for surgery to correct a hernia.

An employee and coworkers performed lockout/tagout procedures on a dryer rotary feed to clear a clogged plug. The employees cleared the plug and began to run the dryer, but the plug had not been placed back into the dryer. The injured employee was pointing to the rotary feed to remind the lead person to put the plug back in when the rotary feed amputated his right index finger from the fingertip to the bottom of the nailbed.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.
