An employee was working on the ground approximately 15 feet from a transmission structure when a knee joint fell from the transmission structure, hit an adjacent apartment building, and bounced onto the employee, striking him in the hard-hat. The employee was knocked to the ground, striking his face and resulting in an eye socket fracture.

A crew had just installed lightning arresters on a de-energized distribution line. When they energized the line, they realized that one lightning arrester needed adjustment. A lineman went up in a bucket to adjust it. When he tried to loosen its bolts, an arc flash occurred between the bolts and his impact wrench. He suffered second-degree burns to both hands and forearms.
A crew was changing out a transformer. The transformer had been disconnected from the conductor. When the hardware affixing the transformer to the pole was loosened, the pin insulator on the overhead conductor broke and the conductor contacted the transformer, which then became energized. An employee who was working on the pole suffered burns and was hospitalized.
An employee was installing an electrical system neutral when the wire came into contact with the energized side of an insulator, creating an arc flash. The employee received electrical burns to the hands and was hospitalized.
