An employee was driving a forklift to put away pallets. After offloading a pallet, he made a wide left turn to head into another building. The lift overturned and made contact with a guardrail, pinning the employee's left leg against the rail and resulting in an injury to his femur.

An employee was doing maintenance on a wall-mounted exhaust fan. The metal fan blade struck the employee, lacerating the employee's right elbow.

An employee was helping a coworker transport a box sofa to a customer's car with a four-wheel dolly when he tripped on the dolly wheel and fell to the parking lot surface. He injured the left side of his body and suffered a bulging lumbar disc, requiring hospitalization.

An employee was pulling a pallet of water when she stopped on a ramp with the pallet. The pallet continued to move and rolled onto her leg, fracturing her left foot.

An employee was performing preventive maintenance on the conveyor motor and amputated the tips of the right thumb and index fingers.

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.

An employee went to step off material handling equipment to grab their jacket when the throttle of the vehicle engaged. The vehicle reversed and the employee's left foot was crushed between two material handlers, resulting in a fracture.

An employee was working at a height of approximately 20 feet and was standing outside his forklift's basket attachment. His forklift was struck by another forklift, causing the employee to fall to the warehouse floor below. The employee sustained facial injuries, broke two ribs and an arm, and also suffered a head injury. The employee was wearing a harness at the time but it was not connected to an anchor point.

On November 8, 2023, an employee was operating a stand-up pallet jack in the warehouse. As he was backing up, the pallet jack struck the basket of a parked aerial lift. His right ankle became caught between the pallet jack and the aerial lift basket resulting in a fractured ankle.

On October 26, 2023, at approximately 12:00 PM, an employee operating a four wheeler was on their way to an off-property break location. The employee stopped the four wheeler near the administrative office to allow a log truck to exit the property. The log truck backed up, and the employee's left ankle was caught between the log truck trailer and the four wheeler, resulting in a fracture and laceration.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was on a 6-foot A-frame ladder, working to replace ceiling tiles. They fell to the ground and sustained fractures to their hip and femur. The employee was hospitalized and required surgery.

An employee was walking backward with a pallet jack loaded with cases of toys. When the employee went to stop the pallet jack, it rolled over his right foot. The employee was hospitalized with a fractured distal tibia and a torn tendon in his foot that required surgery.

An employee was operating a rolling container carrier (RTC) vehicle in pedestrian mode. When he operated the hand lever to turn it, the RTC pinned his left leg against the pick location. The employee suffered a laceration on his left knee and was hospitalized.

An employee was walking in a dispense area when she tripped over a pallet and fell to the ground, resulting in a broken left hip that required hospitalization and surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
