Final narrative
An employee was doing quality control when they stepped back and tripped over a portable roll pusher and fell to the ground, resulting in a broken ankle that required surgery.
On August 31, 2024, at approximately 12:10 AM, an employee was correcting an alignment issue with a stack of pallets that were loaded in the pallet dispenser. His left index fingertip was pinched between the pallet support plates (flapper) and the flapper stop. The employee sustained a fingertip amputation.
An employee was threading paper into the padded bag line machine. When the paper passed through the notcher section, the employee attempted to support the paper with his hand. The notcher fired and amputated his right ring fingertip. The notcher was not turned off at the time of the incident.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
On July 22, 2025, an employee was cutting/torching a large piece of metal when a piece of the material broke loose, struck him mid-stomach, and knocked him to the ground. The employee was hospitalized with an umbilical hernia and fractures to his right femur and left fibula.

The upward motion of the crane caused the cable to pull his finger into the pulley. The employee's fingertip was partially amputated.
The employee was operating a peeler machine when his left hand got caught against the log that was being peeled. The employee was hospitalized with lacerations to the fingers on his left hand that required stitches.
