An employee was standing on the elevated platform at the wood breaker machine. The platform grate became detached and dropped 7 feet to the floor with the employee standing on it. The employee was hospitalized for 3 broken ribs, a punctured lung, and a lacerated spleen and required surgery.

Clarion Boards LLC

Two employees were in an aerial lift, approximately 25 feet from ground level. The employees were spraying water with a fire hose into ductwork on a machine in order to remove a blockage. When the blockage was removed, steam from the ductwork enveloped the employees in the lift. The employees were hospitalized due to steam burns.
