An employee was leaving work when she tripped and fell, striking her head on the escalator and her arm on the floor. The employee was hospitalized.

At about 6:45 a.m. on April 10, 2021, an employee was scraping down the inside of a mixing bowl when the mixer dough hook caught his index finger. He suffered a broken finger and a cut.

An employee was attempting to remove a stuck towel that was caught in a bin dumper machine when her finger was caught between the bin and bin dumper machine, cutting and lacerating her left index fingertip.

On October 11, 2025, an employee was walking through a parking lot when they tripped on a pavement joint and fell, resulting in a fractured right ankle and a left knee injury.

An employee was unloading merchandise from a pallet when she tripped over the pallet and fell forward. Her head and face struck the floor of the stockroom, resulting in multiple head and face injuries that required hospitalization.

His right hand and two fingers got caught between the rear wheel of the cart and the coaster rail. His ring and little fingers were crushed and lacerated, resulting in surgery and partial amputation of the ring finger.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
