APM TERMINALS

An employee developed heat-related illness while working at a freight terminal, removing items from a container. Dehydrated and suffering from heat exhaustion, the employee lost consciousness.

An employee was tightening a turnbuckle while attempting to conduct lash-back operations on a ship when their left gloved hand was caught between the upper and lower turnbuckles. The employee suffered a crush injury to the little finger that resulted in a fingertip amputation.

An employee was fixing a hatch cover on a vessel when the hatch shifted and crushed their right little finger between the cover and the deck. The employee sustained a fractured finger.

At around 4:45 p.m. on August 13, 2020, an employee had just parked a rubber tire gantry and was climbing down from it. He slipped and fell from the fourth rung to the ground, suffering a broken leg and a possible broken tibia and fibula.

A truck knocked over an employee, who suffered broken bones.

An employee was driving a forklift when it collided with another forklift. The employee's middle finger was pinched between the two forklifts and partially amputated.

An employee was operating an ATV when they were struck by a vehicle. The employee sustained multiple bruises and internal injuries.

A temporary employee was operating a powered industrial truck and attempting to maneuver around another automated powered industrial truck when the vehicles collided. The employee sustained a crush injury to their right foot.

An employee was operating a double pallet jack down an aisle when he was struck by another double pallet jack. The employee's right lower leg was then caught between the pallet jack and a rack beam, resulting in an injury just above the right ankle that required hospitalization.

An employee was operating a ride-on double pallet jack when he was struck from behind by another pallet jack. Their lower right leg was caught resulting in fractures to the tibia and fibula.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.

An employee had been loading packages into a vehicle. While exiting the vehicle, he tripped and fell, fracturing both ankles.

An employee was working from the elevated bucket of an excavator when they fell between two tanks and landed on the ground. The employee sustained a fractured leg.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
