An employee was attempting to move a steel form panel with a crane when the spreader beam became tangled with a rebar cage. As the load began lifting, the tension from the chains making contact with the rebar cage allowed the load to swing toward the employee, trapping him between the steel form and a solid staircasing and resulting in an injury to his left leg.
While dismantling a mold, an employee's left hand fingers were in a hole and one of the metal panels moved, crushing and amputating their left middle fingertip.
