On October 28, 2024, an employee was using a press to bend a piece of bar stock. The part slipped, causing him to fall to the concrete floor and hit his head. The employee was hospitalized with a contusion to his head and a laceration on his arm.

An employee was blowing bearings from underneath a ball bearing assembly machine using an air hose. The air hose nozzle was pulled under the machine table, which rose and pinched the employee's right hand against a structural support. The employee suffered a partial amputation to the right thumb.

An employee was placing hinges on a fixture. A blade activated and came down while her right hand was on a hinge, amputating three of her fingers.

An employee was operating a mechanical power press and amputated the right index, middle and ring fingers.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

An employee was working on a portable stainless-steel vessel equipped with a butterfly valve at the bottom. The employee used a crane to raise the vessel and connected an airline to the valve control. Upon activating the compressed air supply, the valve opened. While holding the bottom of the vessel with their left hand, the airline disconnected and the valve closed on their left index fingertip. The employee sustained an avulsion of the fingernail, an open nondisplaced fracture of the distal phalanx, and partial amputation of the fingertip.

An employee was in training to learn how to change steel dies on a culvert machine. The employee was on top of the machine when the handheld control was activated, engaging the machine's auto-run function. The employee's feet were dragged into the roller dies, resulting in a left ankle fracture, a laceration between the ankle and knee, and amputation of the the big toe and second toe on the right foot.

An employee was walking up the stairs in an office area. Their right foot caught the edge of the stair at the top of the stairwell and they fell forward. The employee sustained a neck fracture.

An employee was operating a clipper machine and contacted maintenance when they noticed the machine was not working properly. Maintenance worked on the machine. While verifying that the machine was fixed, the employee's left index finger was amputated above the first knuckle.

An employee was feeding a strip of steel into a power press when it activated and contacted the injured employee's left hand. The employee suffered amputations of the first two digits of the hand.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.
