An employee was feeding a strip of steel into a power press when it activated and contacted the injured employee's left hand. The employee suffered amputations of the first two digits of the hand.

Norflex, Inc.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

Galaxy Balloons Inc.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

ERICO International Corporation

An employee was working on a portable stainless-steel vessel equipped with a butterfly valve at the bottom. The employee used a crane to raise the vessel and connected an airline to the valve control. Upon activating the compressed air supply, the valve opened. While holding the bottom of the vessel with their left hand, the airline disconnected and the valve closed on their left index fingertip. The employee sustained an avulsion of the fingernail, an open nondisplaced fracture of the distal phalanx, and partial amputation of the fingertip.

Cardinal Hardware & Lumber, LLC

An employee was in training to learn how to change steel dies on a culvert machine. The employee was on top of the machine when the handheld control was activated, engaging the machine's auto-run function. The employee's feet were dragged into the roller dies, resulting in a left ankle fracture, a laceration between the ankle and knee, and amputation of the the big toe and second toe on the right foot.

Stanley Black and Decker

An employee was walking up the stairs in an office area. Their right foot caught the edge of the stair at the top of the stairwell and they fell forward. The employee sustained a neck fracture.

An employee was operating a clipper machine and contacted maintenance when they noticed the machine was not working properly. Maintenance worked on the machine. While verifying that the machine was fixed, the employee's left index finger was amputated above the first knuckle.

United Refrigeration Inc

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

Michels Power, Inc.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

Parker-Hannifin Corporation

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
