At 9:45 a.m. on May 3, 2021, an employee was sitting in front of an iron plate, using a hand grinder to remove tack welds from it. The wheel jumped, slipped on his right leg, and cut him just above the knee on the side. He was hospitalized.

An employee was standing on a sheet of -inch by 4-foot by 8-foot plywood on top of a rebar mat, supervising his crew.
