On April 3, 2017, at 11:55 a.m., the injured employee and three other employees were transporting a decommissioned, pumped out, triple rinsed steel metal tank, 4' x 5' x 4' using a forklift to a metal recycling roll-off container located outside behind the plant. The injured employee was guiding the tank with his hands when the rest of the crew was attempting to place the tank into the roll-off container. The tank slipped off the forks and crushed the injured employee's left ring fingertip pad between the tank and metal container wall, amputating the left ring fingertip pad.

On August 26, 2025, a temporary employee was assisting a mechanic. The employee was on an overhead crane, removing a chain hoist attachment over a large plating tank. When the platform of the crane was moved, the employee became caught between the platform's railing and a support beam. The employee sustained a punctured lung and fractures to their clavicles, humerus, and ribs.
