Final narrative
A piece of duct fell from a duct jack approximately five feet. The injured employee attempted to catch the falling piece of duct and crushed his fingertip between the duct and a nearby support steel frame resulting in a partial fingertip amputation.
An employee was using a fixed ladder to take copper piping to the roof when he missed a rung and fell approximately 6 feet. He was hospitalized with fractures to his neck, back, and pelvis.

After performing the shutdown, he went through a set of double doors and onto a small walkway that led to the mechanical room of the elevator.
