Final narrative
An employee was shutting down a riser pipe to a fire sprinkler system in a section of a building. After performing the shutdown, he went through a set of double doors and onto a small walkway that led to the mechanical room of the elevator. The elevator's floor had been removed as a result of a construction project, and the employee fell approximately 80 feet down the shaft. He suffered spinal fractures, leg fractures, and a broken arm.

A piece of duct fell from a duct jack approximately five feet. The injured employee attempted to catch the falling piece of duct and crushed his fingertip between the duct and a nearby support steel frame resulting in a partial fingertip amputation.
