An employee tripped over a piece of angle iron that was on the floor. He landed on his left knee and the knee was fractured.

An employee was sandblasting metal and dropped the sandblasting gun, causing sand to strike their left triceps.

An employee was using an overhead bridge crane to position a piece of steel on a band saw. The employee went to stabilize the piece when the hydraulic clamp closed on the employee's little finger. The finger was surgically amputated at the first knuckle.

An employee was sitting on a forklift while on break in the paint shop. An unknown person/assailant walked into the shop and shot the employee in the head.

An employee was using an overhead crane to dump blasting media in a 55-gallon drum into a dumpster. The chain slipped and a pinch point was created between it and the drum. The employee's thumb was caught, resulting in a right thumb tip amputation.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

On October 21, 2025, an employee was operating a saw when a pneumatic clamp caught their right middle finger. The employee suffered a partial amputation to the finger.

An employee completed an end cut on a metal shim and started transferring the metal shim from the saw conveyor to two saw horses in the area. The employee set one end of the shim (7 feet long x 8 inches wide, weighing 218 pounds) on one of the saw horses, then slid the other end onto the other saw horse and the shim fell from the first saw horse. The employee's left index fingertip was pinched between the saw horse and the shim, resulting in a skin avulsion. The fingertip was amputated later.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was moving steel beams with an overhead crane when the bundle struck a bollard and became unstable. As the employee reached to stop the material from shifting, his thumb was pinched in-between the beams, resulting in a partial amputation.

An employee was rigging a steel joist to be lifted by a crane. The joist fell and pinned the employee against the concrete ground. The employee sustained fractures to multiple ribs, his right arm, right tibia, and an open fracture to his right ankle, as well as a dislocated right hip.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
