An employee was using a circular saw to cut a 2 x 4 that was attached to a concrete form. They went to catch the off-cut material, and their left hand contacted the blade of the circular saw. The employee sustained the amputation of their ring finger and amputation of the middle finger including both knuckles.

An employee was bringing in a split bearing when his finger was caught between the bearing and the eye bolt that was holding the bearing. The employee's fingertip was amputated.

On December 1, 2022, an employee was in the process of taking down a damaged overhead garage door frame, when the door frame fell on their arm, resulting in a fracture requiring hospitalization.

An employee was walking during a demolition job when he tripped and fell onto the steel decking of the working surface he was on. He sustained several fractured ribs on the right side of the body, an injury to his right lung, and a laceration to the liver.

An employee was struck by an air hammer chisel, breaking a bone in his right foot.

An employee was power washing a metal tank from an elevated platform. They fell through a gap between the platform and the tank. The employee landed on the ground and sustained fractures to their ankles and feet.

An employee was replacing an HVAC unit in a residential home. While he was in the attic space above the garage, the wood he was walking on broke, causing the employee to fall through the sheetrock to the concrete floor below. The employee bumped his head and sustained a shoulder injury as well as a broken ankle.

An employee was putting a tarp on a water tank while standing on a trailer. She lost her footing and fell through the trailer bed, striking the corner of the trailer bed. The employee sustained internal bleeding.

An employee was repairing a sub-floor in a residential house that had water and fire damage. The employee fell through the sub-floor and landed in the basement, resulting in spinal fractures that required hospitalization.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
