An employee was separating a concrete form from a vault. The form came off the hoist hook, and the employee's fingertips were crushed between it and the vault. The employee's right ring and middle fingertips were medically amputated.
Two employees were preparing to pour concrete to manufacture burial vaults. The belt on the concrete auger broke. One of the employees had replaced the belt and was fixing a twist in it, with his left middle finger under it, when the machine was turned on. The fingertip was amputated about 1/4 inch below the nail.
