The injured employee was removing labels from boxes. A forklift was driving to pick up boxes and the driver stopped to talk to the injured employee. After the driver spoke with the injured employee, the forklift reversed and ran over the injured employee's right foot, crushing it. The employee's foot was surgically amputated midway up the foot.

When the fan was turned on to verify the direction of its rotation, the vacuum of the fan pulled the tip of the employee's right index finger into the spinning blade, causing it to be severely lacerated from the middle of the nail to the end of the finger.
