During routine maintenance of the bonnet end cap on a blowout preventer, the cap fell and smashed an employee's right ring finger against another ram bonnet. The finger was amputated.

An employee was positioning an oilfield drill bit when it fell from the connection fixture it was connected onto. The bit fell onto the employee's left ring fingertip and crushed it against the work surface. The fingertip was medically amputated.
An employee was acting as a spotter and guiding a flatbed operator while standing on the rear passenger side of an outrigger leg when he stepped back onto a mobile crane platform and fell approximately 70 inches to the ground. He was hospitalized with an open fracture to the left ankle.
