An employee was using a pallet jack to maneuver boxes of merchandise from a box van to a lift gate when the pallet began to roll and the jack handle pushed the employee off the lift gate, resulting in a possible broken ankle.
An employee was carrying a plastic tote full of brake rotors. His foot caught the corner of a pallet, causing him to fall to the concrete floor and break his hip.
An employee was on a 6-foot ladder, getting a boxed cooling fan from a top shelf in a storage area. His hand slipped off the box, causing him to fall backward from the ladder to the concrete floor.
