On July 3, 2021, at approximately 2:00 p.m., an employee parked a forklift next to another forklift. While exiting the forklift, the controls engaged causing the forklift to move and crush the employee's left leg between the two forklifts resulting in hospitalization.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.
