An employee was closing a roll-up trailer door when he lost his footing and the door closed on his right leg. The employee was hospitalized with a fractured right leg.

An employee was performing regular observation of the gate and all approaching vehicles. The weather was hot and the employee was sitting in a vehicle. The employee sustained dehydration and heat exhaustion.

On April 30, 2025, a driver was making local deliveries. At approximately 10:15 AM, while climbing down from the back of the trailer, he lost his balance and fell to the ground from a height of less than four feet. The employee's left leg was fractured and he was hospitalized.

On January 25, 2025, at 6:36 a.m. an employee was opening the door of a trailer at the dock. The 55-gallon drums loaded inside had shifted during transit, causing them to lean against the trailer door. The drums fell when the door was opened, and the employee was crushed. The employee was hospitalized with a broken right tibia.

An employee was using a dolly to hook two trailers together. When she went to move the dolly, she felt pain in her back and suffered a back sprain that required hospitalization.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
