On 7/13/2021, at approximately 4:55 PM, an employee was lifting a drain grate to remove a glove from the drain when the grate slipped and fell on his left hand causing the amputation of the left middle finger at the first joint behind the fingernail.

At 4:58 p.m. on July 10, 2023, an employee removed a cover on a neck peeler machine to clean meat residue from the machine. A sprocket snagged the employee's jacket and pulled it into the machine, lacerating the employee's left upper arm and hand. The employee was hospitalized.
An employee was rinsing a food processing machine at a facility and went to stabilize himself. His left hand contacted the surface of the machine and a guard inadvertently closed, crushing the tip of his ring finger and resulting in an amputation and fracture.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
