On 7/13/2021, an employee was carrying a conveyor roller while walking backward on a catwalk. The employee missed a step while going down the stairs, causing the employee to fall backward. The roller came down onto the employee's fifth digit on the right hand crushing it against the catwalk decking and fracturing it.
An employee was tightening a clamp (using channel lock pliers) on a pasteurizer system when a leak was identified. The clamp fastener dislodged while turning and 240-degree liquid discharged onto the employee, causing burns to the waist, abdomen, legs, and knees.
An employee was cleaning a packaging machine when the machine started. The employee's right index finger was then caught between the machine's sealer bars, resulting in amputation.
