An employee was working with precast forms. A form was lowered and struck the employee's left hand, dislocating and lacerating his knuckle. He was hospitalized.

On Monday, September 14, 2020, at approximately 10:40 AM, employees were setting precast concrete wall panels (45' x 12'). A lift was used to pickup braces. The frame of the lift contacted a wall brace causing a concrete panel to fall. As the panel fell, it struck a manlift that was being used to install braces. As the manlift fell, it struck the lift causing an employee in the manlift to be ejected. He was wearing a personal fall arrest harness at the time. The employee sustained a fractured right femur and fractured ribs and was hospitalized.

An employee was threading a steel cable through a stress head when it became obstructed. The employee looked in the equipment to see what the obstruction was when the tension released and the wire struck him in the eye.

An employee was checking the function of newly installed actuators when they were struck by a pressurized release pipe. The employee suffered fractures to the orbital socket and shoulder, requiring hospitalization.

An employee was removing chains from a steel tube. A sorting hook attachment was caught on the staging area cross member. When he raised the chains, the sorting hook snapped back and struck the employee's face causing a fractured jaw.

Southern Parallel Forest Products Corp.

An employee was in the sawmill basement using a push broom and shovel to clean wood chips and sawdust. The employee's right arm was struck by a wooden board between the wrist and elbow, resulting in a fracture.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

Red Dot Corporation

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

Cornell & Company, Inc.

An employee was standing on the surface of a steel beam about two feet wide. He slipped and fell backward, landing on the concrete ground about 20 feet below. The employee sustained a broken pelvis.

K Constructor, LLC

An employee was unloading a rebar delivery from a flatbed, 19-wheel trailer. The employee rigged the load with wire rope chokers. The load was picked up 3-4 feet above the trailer deck by a crane. The load began to swing and the employee grabbed onto it to prevent it from being pushed off the trailer deck. The crane continued to lift the load, and the employee let go so the crane did not lift him to the third story. The employee fell approximately 12 feet to the ground below, resulting in a back injury and fractured vertebrae.

Bowman Steel, LLC

An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
