On July 20, 2021, at approximately 8:00 a.m., an employee was on a 6-foot high rolling platform ladder hanging 10-foot metal hooks on a chain. The employee was hanging a hook when one of the hook ends got caught in the handrail of the rolling platform ladder and tipped it over. The employee fell to the concrete floor and sustained a contusion to their side.
An employee was moving a rack of 25 20-foot steel channel stacks, weighing 625 pounds, on a cart. The cart got stuck on a crack on the ground and tipped over; the channels fell onto the employee's left leg and broke it.
An employee was removing parts from a rack while standing on a portable platform. He lost his balance and fell approximately 50 inches to the walkway below, breaking two fingers and fracturing his pelvis.
