A temporary employee was training on a stand-up forklift. The forklift spun and pinned the employee's left leg against some racking, and he struck his head against the racking. The forklift then ran over his left foot, crushing the ankle and resulting in an amputated foot.

On 3/25/2021, an employee was driving from the warehouse to a designated lot space. While turning, the trailer became unstable and began to tilt and the cargo container detached from the chassis. This motion forced the tractor to rise off the ground. As the container slid off the chassis, the tractor came down with force and bounced four times before coming to rest. The employee struck their face on the steering wheel and was hospitalized with multiple fractures to the face and head, a laceration above the left eye, and bilateral eye bruising/swelling.
