An employee was climbing down a ladder from a platform on a pressure vessel. The employee fell at the second rung of the ladder, landing on the floor and suffering a broken left elbow. He was hospitalized.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
