At about 12:45 a.m. on August 6, 2021, an employee was trying to dislodge paper stuck inside a rotating valve on a bagger. The blade in the valve was activated, rotated, and amputated the employee's right index and middle fingers.

Employees were conducting an annual separator service for a customer. An engineer was replacing a sliding bowl into a separator when his left thumb was pinched between the sliding bowl and the bowl body as a crane lifted it. His thumb tip was amputated above the bone.

An employee was adjusting plates on a printing press and caught a finger in the gears resulting in a partial amputation to his left middle fingertip.

When they stepped on the paper, they slipped and fell to the floor, twisting their right ankle. The employee was hospitalized with a broken ankle that required surgery.

An employee was performing preventive maintenance on a drive belt. The drive belt moved and the employee's fingers were pinched between it and a sprocket. The employee's left index fingertip was amputated and their left middle finger was lacerated.
