An employee was walking around a stack of tubs when her right toe struck a dolly and she fell to the floor. She suffered a broken left hip.
An employee was cleaning racks inside a warehouse at ground level when a pallet on the third level of a nearby rack failed, allowing a large load of wrapped cookies to fall on him and fracture his left femur.
At approximately 3:45 AM on April 1, 2015, at the Interbake Foods LLC bakery in North Sioux City, South Dakota, the injured employee was helping other employees move a rotary molder machine by hand. While moving the machine, his right index fingertip was amputated halfway down his fingernail by the hinge on a 2' x 2' control panel box attached to the rotary machine.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was using an air hose to blow built-up dough off the rollers under the cutting apron on a conveyor belt line.
