At 10:30 a.m. on August 26, 2021, an employee was helping unload material from a flatbed trailer. He was standing on product when he tripped and fell, striking his head on the ground and losing consciousness as well as suffering two broken ribs. He was hospitalized.

At 8:30 a.m. on April 24, 2021, an employee was on the fifth rung of an 8-foot ladder. The ladder was knocked over and he fell to the concrete floor, suffering broken bones in his left leg and tendons in his left shoulder.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
