Final narrative
An employee was at the extruder pushing pipe down the line. As the employee was manually positioning a piece of pipe that was being heated, his finger was caught in an unguarded rotator chain resulting in an amputation.
A machine used to form PVC pipe began to jam up; the formed pipe began to bow. An employee cut the pipe, and a piece of it flew into another employee. That employee suffered a fractured left eye orbit, fractures to facial bones on the left side, and three fractured left-side ribs.
