A temporary employee fell from an A-frame ladder to the floor about 5 to 6 feet below. He suffered fractures to his left hip and was hospitalized, requiring surgery.

A temporary employee was operating an electric pallet jack at a loading dock. While maneuvering the pallet jack into a trailer, the equipment loaded onto the pallet jack began shifting to the left.
