An employee was walking on uneven ground that was covered in a mixture of ice, snow, and slush. He slipped on the ground and fell, fracturing three ribs. The employee was hospitalized.

An employee was using a vacuum pump and a 2-inch hose on a tanker truck to unload hot oil from a tank located inside a customer's facility. The hose became clogged and kicked up and out of the tank hatch, spraying the employee with hot oil. The employee suffered second-degree burns to their abdomen.

An employee was stepping out of a car when they slipped and fell on ice, resulting in a femur fracture.

An employee was closing up for the weekend when they slipped on oil and brake fluid and fell. The employee suffered a leg fracture.

An employee was working in the forest flagging an area to be harvested. He took a step and his foot slipped on a stick hidden under the leaves, causing him to fall to the ground. He landed on his right foot/lower leg resulting in a fractured tibia.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was riding an all-terrain vehicle (ATV) in a field when the ATV tipped over. The employee fell off the vehicle and sustained a fractured hip.

An employee was inspecting an air conditioning wall unit in the warehouse when he fell through a secondary roof 8-9 feet to the floor. The employee sustained fractures to his L1 and T12 vertebrae as well as a head injury that required staples.

On July 29, 2025, at approximately 3:00 PM, the injured employee was managing hand pollination operations in a corn research nursery field. They were working outside and riding in a side-by-side with two other employees. They were overcome with heat exhaustion, lost consciousness, and fell in the break area of the nursery. The employee was hospitalized.

An employee was performing a routine site visit when he was struck by a car while on the shoulder of a road. The employee was hospitalized with two fractures to his leg.

Employees were drilling a hole when an employee was struck by the drill. Their right leg was amputated at the knee.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
