Final narrative
On October 11, 2021, an employee was installing a new tank into a pit when the strap holding the tank broke. The tank rolled into the pit and the employee was crushed between the wall of the pit and the tank, resulting in contusions and a broken hip.
An employee was setting up to move chemical from a tank to a drum when a hose came loose and chemical splashed on the employee, burning the upper left arm and left side of the body. The employee was hospitalized.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.
