An employee was on the upper half of a track ladder that was attached to the aisle and put a box onto a shelf. He fell to the floor and landed on his back, resulting in a back fracture and a concussion.

An employee was pushing a cart through a parking lot on the crosswalk. A vehicle struck the employee, who suffered a skull fracture, a mild concussion, and a laceration.

An employee was unloading a docked trailer when she tripped on a pallet that was partially unloaded and fell forward into the pallet. The employee struck her forehead and the bridge of her nose. She was hospitalized with contusions to her nose and forehead, as well as two slipped discs in her neck. The employee was hospitalized for surgery.
