An employee was carrying a box of blinds (approximately 36 pounds and 6 feet long) down a small flight of exterior concrete stairs at the customer s preferred delivery location. He fell down the last two steps to the concrete ground below. The employee sustained a dislocation of his right knee and a torn patellar tendon, requiring hospitalization and surgery.

An employee was operating an electric pallet jack (EPJ) and making a U-turn when they became caught between the pallet jack and racking. The front part of their foot was pinned between the racking corner guard and the EPJ platform. The employee sustained lacerated tendons to the right big toe that required surgery.

On September 18, 2023, an employee was moving pallets of sinks with a double electric pallet jack when their left foot was pinned between the pallet jack and another pallet. The employee's left ankle was broken.

An employee was offloading materials to deliver to a customer when they fell from the back of a box truck to the pavement. The employee sustained a clavicle fracture, fractures to ribs 2-9, and a head laceration.

An employee was pulling a bifold door from an aisle in the warehouse. While transporting the door to another location, the door slipped, cut his right hand, and hit his left ankle, causing it to swell. The employee sustained a broken left tibia.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was walking through a parking lot to his car while servicing a customer when he slipped and fell on ice, resulting in a fractures to their elbow and knee.

An employee was moving a light fixture when a wire nut fell off, leaving live wires exposed. The employee contacted the wires and was shocked.

An employee was on a ladder putting up a poster on a glass front. The ladder started to slip. He jumped off, landed on his arm, and suffered a broken elbow.

An employee slipped on ice/snow and fell in the parking lot while accessing a work truck, suffering a back/rib injury.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
