An employee was exiting a coal conveyor when he stepped on a piece of plywood that broke, causing him to fall approximately 80 feet into a bunker. The employee suffered a broken wrist, a head laceration, and brain bleed.
On 10/25/2021 at approximately 10:25 a.m., an employee was pushing a catalyst when they fell 41 feet through a gap in the grating, resulting in a lacerated spleen, lacerated kidney, and bruised shoulder.
An employee torch-cut an expansion joint around a piece of ductwork at a cage mill. After removal of the last piece of expansion joint, the top pipe chute came down and caught the employee's right arm between the top pipe chute and the bottom pipe chute. The employee sustained a crushing injury to the right arm, requiring hospitalization for surgery.
An employee was torqueing a bolt with an impact gun. The employee's finger was caught between the bolt and socket when the impact gun trigger was engaged, resulting in a left index fingertip amputation.
An employee was removing a re-framed wall stud section on the fourth level of a building. He stepped backward and fell 40 feet down a grease chute to the concrete floor. The employee was hospitalized with swelling and bleeding above his right eye and internal injury to the pancreas.
On 1/06/2023, an employee was on the fourth floor flat roof of an apartment building construction worksite. The employee was cleaning up nails and trash and putting them in a trash bag. The employee fell through a hole on the fourth floor and landed on the concrete floor of the first floor. The employee sustained a broken right ankle and required back surgery.
An employee was putting a tarp over a leak in a concrete roof. The employee fell through the roof to the floor 32.5 feet below, suffering a broken collarbone and leg, as well as a spinal injury that required a pin.
A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.
An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.
An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.
An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.
An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
