An employee was processing vehicles when a vehicle did not stop directly in front of his station. The employee stepped onto the vehicle track to retrieve paperwork from the vehicle. The employee then slipped on the track and fell approximately 5 feet onto the ground below, resulting in a facial laceration as well as a cheek bone fracture.

Employee 1 was using a forklift to unload engines and transmissions from a truck. The forklift couldn't get to the engines into the back of the truck, so employee 2 moved the engines to the edge of the truck with a pallet jack. Employee 1 reached to grab the pallet while employee 2 pushed with the pallet jack. When the pallet hit the sheet metal strip of the truck, the engine shifted onto employee 1's finger, resulting in a partial amputation.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was conducting torching operations using oxygen and propane to cut metal beams within the recycling laydown area. The employee used a hose with an air chuck connection to fill up the yard pickup truck's back tire with oxygen. The tire blew up which caused the oxygen to ignite. The employee sustained fractures, internal injuries, burns, and lacerations.

Aesop Auto, LLC

On April 22, 2025, an employee was loading a transmission onto the truck. He went to manually adjust the forks of a sit-down forklift when the transmission fell. The employee's right middle finger got caught between the concrete and the fork, resulting in traumatic amputation of the distal phalanx.

Bay Auto Parts, Inc.

An employee was in the back of a delivery truck when a pickup truck struck the delivery truck. The employee contacted the side wall of the delivery truck and sustained a head injury.

An employee was moving a metal crate of unstable product on a rack so that it could be removed, when the crate became dislodged and fell. The crate struck his left ankle and his lower leg was fractured.

A salvage vehicle was on a front-end loader/forklift with the forks completely down. The operator got out of the loader to retrieve something from the salvage vehicle when the forks on the front-end loader began to lift the vehicle up while he was in it. He went to step out of the vehicle and fell five to six feet to the dirt pad below. The employee sustained fractured hip and pelvic bones.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
