An employee secured her pallet jack on a trailer, then turned the handle to lock it. The jack moved and pinned her right leg against the side of the trailer. She suffered a broken tibia and fibula.

An employee was unloading a pallet jack from a flatbed trailer. When the tire of the pallet jack contacted the wood decking of the trailer, the jack shifted abruptly and caused the employee to fall off the side of the trailer, landing on the ground.
