On November 16, 2021, an employee was loading product onto a truck at the warehouse loading dock. While unjamming the dock plate, his right middle finger was caught between the dock plate and the trailer's threshold, resulting in amputation of his right middle finger at the nail.

An employee secured her pallet jack on a trailer, then turned the handle to lock it. The jack moved and pinned her right leg against the side of the trailer. She suffered a broken tibia and fibula.
