Employees were performing an upgrade to a simulator device. They had to access an attic area so they could run new electrical cables through the ceiling. The building had 2x6 boards laid in this area to allow someone to work without having to balance on the ceiling joists. An employee was walking on one of the 2x6 boards when it shifted. The employee fell through the ceiling to the floor approximately 12 feet below. The employee was hospitalized with multiple fractured ribs, a fractured humerus, a partially collapsed lung, and an injured spleen. Surgery was required for the fractured humerus.
An employee was driving an aerial lift onto a trailer while at an auction yard when the trailer moved, catapulting him from the aerial lift and onto the trailer. The employee suffered a broken right femur, a broken hip, and head trauma.
