On November 11, 2021, at approximately 5:45 AM, a temporary employee tripped over a wheel on a smock holder and fell to the floor. The employee was hospitalized with a hip fracture.

On July 7, 2025, an employee was cleaning food from under a conveyor belt. He was retrieving a piece of food when his left middle finger contacted the chain and pulley. The fingertip was amputated. The machine was guarded at the time of the incident.
On December 16, 2024, at approximately 3:07 p.m., an employee was feeding frozen fish blocks via a conveyor belt into a floor mounted electrically powered grinder. The grinder became jammed with the fish material. The worker deenergized the machine and used a rolling ladder to climb approximately 4 feet in the air to unjam it. As the fish block became dislodged, the momentum of the action caused the fish block to slam down on her right thumb and the machine, causing a partial amputation of the thumb tip.
On January 29, 2024, an employee was accessing a bagging machine from beneath to clear a jam. Her hand was caught in the machine's point of operation, where a heat sealer bar and cutter amputated her left index finger at the first knuckle. The machine was operating at the time of the incident.
On November 4, 2023, an employee was unclogging an oil line for a fryer and began by closing the valves at the fryer drain, pump, and intersection in the pipe. This caused the short section of the pipe to become pressurized. As the employee opened the valve at the bottom of the fryer, the air in the pipe blew up into the fryer full of hot oil, causing it to overflow onto their hands. The employee suffered second-degree burns to both hands.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.
