An employee was wrapping a pallet to be shipped. As he was walking around the pallet with a roll of shrink wrap, a forklift ran over his right heel and broke it.

On March 1, 2023, at 2:34 a.m., a maintenance employee responded to a work order on a conveyor belt that was making noise from the line. The employee removed the belly pan from underneath the conveyor system to visualize and troubleshoot it, and then left to clean the pan. When the employee came back to the conveyor line, he saw some debris under the roller where he had removed the belly pan. He attempted to brush it off when his right hand became caught in-between the roller and the conveyor frame, resulting in abrasions to the top of his right hand.

On August 22, 2022, an employee began experiencing neck and right shoulder pain during her shift after performing job-related tasks such as repetitive lifting. She was hospitalized for a right shoulder sprain and a pinched nerve.

On April 14, 2021, an employee was using a manual pallet jack when she saw a stand-up pallet jack turning into her aisle. She stepped back, tripped over her jack, and fell to the floor. She suffered a broken right wrist and was hospitalized.

An employee was operating a reach truck. As the employee steered into an aisle, the reach truck was headed for a rack and the employee jumped off the truck. His left leg was pinned between the truck and the rack, resulting in hospitalization for a broken left leg that required surgery.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was on a platform, trying to fix a pulley belt in a dust cyclone. As he tried to stop the belt, it pulled in his hand, causing the amputation of the tip of his right little finger.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
