A driver was reseating a seated rack in a trailer. As the rack cup was seated onto the post, the palm side of the employee's right little finger was caught between the two. The employee suffered an avulsion to the finger and was hospitalized, requiring surgery.

An employee was performing diagnostics on an automatic elbow machine when the forming piece that runs around in a channel severed the tip of his left middle finger, resulting in an amputation.

An employee was unscrewing fasteners to remove the guarding for a roll former machine to expose two sets of rollers. The employee then placed a flathead screwdriver perpendicular against the edge of one of the wheels in the set furthest away from his position to remove galvanized build-up. During this process, the screwdriver hit the wheel and was pulled into the rollers pinning the employee's left ring finger between the screwdriver handle and the machine. The shaft of the screwdriver then broke, releasing the employee's finger. The employee suffered a soft tissue injury and amputation of the left fourth digit. The machine was powered and operating at the time.

While walking to the lobby, an employee slipped and fell on a wet tile floor. The employee was hospitalized with a fractured kneecap.

An employee was operating a duct line. As the employee was adjusting the feed, the duct line amputated his right little finger. The machine was not locked out/tagged out at the time.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was pulling a steel parts handcart when its wheels caught on something and locked up. The cart fell backward. The cart handle struck the middle of the employee's right foot, breaking bones. The employee was hospitalized and required surgery.

An employee was cutting wood with a circular saw. The saw kicked back, and its blade cut three fingers on the employee's right hand, resulting in an amputation.

An employee was installing a door from an A-frame ladder. He fell from the ladder, landed on his feet about 8 feet below, and suffered a broken tibia.

An employee was leaving the building at the end of the day when she missed a step and fell, fracturing her right foot.

Two employees were running a CNC folder machine. The injured employee went to adjust the metal and the machine came down, crushing her forearms. Her left arm was fractured and both arms required stitches.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
