An employee was manufacturing a replacement shaft for a malfunctioning conveyor. To polish the shaft, the employee placed emery cloth around it while it was turning in the lathe. The employee was caught by their glove and pulled into the spinning equipment, suffering lacerations around the shoulder, neck, and ear; a puncture behind the left ear; and an amputation of the left index finger between the first and second knuckles.

An employee was about to dump sand from a hopper, raised on the forks of a forklift. As the hopper's weight shifted forward and it began to dump, the employee's left middle finger was pinched between the forklift fork and the metal frame of the hopper. The fingertip was crushed, and the employee underwent a medical amputation at the first knuckle.

An employee was dislodging a build-up of semi-molten glass that had accumulated on the outside of one of the rotating cooling rollers on a glass marble machine. While tapping the accumulated glass with a metal bar, the bar jerked downward and pinched the employee's right little finger between the metal bar and the frame of the machine. The employee sustained a partial amputation of the finger just below the nail.
