Final narrative
An employee was dislodging a build-up of semi-molten glass that had accumulated on the outside of one of the rotating cooling rollers on a glass marble machine. While tapping the accumulated glass with a metal bar, the bar jerked downward and pinched the employee's right little finger between the metal bar and the frame of the machine. The employee sustained a partial amputation of the finger just below the nail.

As the hopper's weight shifted forward and it began to dump, the employee's left middle finger was pinched between the forklift fork and the metal frame of the hopper.
